2008 Award Nomination Form

The Shelley Joseph-Kordell Award Steering Committee invites nominations for the Shelley Joseph-Kordell Award. Shelley made professional advocacy and service for seniors her life’s passion, helping them retain their independence and make life transitions more seamless. To honor Shelley’s life and her commitment to seniors, Hennepin County established this Award to annually recognize professionals or agencies who help seniors navigate financial, health care, and social service systems; identify and overcome barriers to community living; and coordinate community resources and support services.

The Award includes a $1,000 monetary gift to the recipient.

The recipient of the Award must be available to attend the Shelley Joseph-Kordell Award and Scholarship Ceremony which is the finale of the Fall Aging Conference held in Brooklyn Center MN on October 26, 2007.
